Assuming you haven't already decided to send your 3rd grader to St Thomas, here is what I suggest:
1. Contact D150 regarding school choice at 672-6744. The letter you have (which is for the NCLB choice program) does appear to be wrong since you are given middle school choices for a third grader. However, I wouldn't get my hopes up for a successful transfer to a different primary school under NCLB - there simply aren't enuff spaces nor enuff schools expected to meet AYP.
2. Complete the form 2Anon posted. That form is for the District's school choice and boundary waiver process (different from the letter you received). That form must be returned by Wed the 8th.
You will find out before school starts if your child is accepted to a new school under the District program. If not, you still have the limited possibility of receiving notification of your child being accepted under the NCLB program a few days after school starts.
